Medical Services
Lotus Direct Care provides direct primary care and functional medicine services. Our services include:
- Primary care consultations and treatment
- Functional medicine evaluations
- Wellness and preventive care
- Specialized treatments as offered
Patient Responsibilities
As a patient, you agree to:
- Provide accurate and complete health information
- Follow treatment plans and recommendations
- Attend scheduled appointments or provide adequate notice for cancellations
- Pay for services as agreed upon
- Respect our staff and other patients
Appointment and Cancellation Policy
Appointments can be scheduled through our patient portal or by calling our office. We require at least 24 hours notice for cancellations. Late cancellations or no-shows may result in a fee. Emergency consultations are available for established patients.
Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept cash, check, and credit cards. For membership-based services, payments are due monthly on the agreed-upon date. Failure to pay may result in suspension of services.
Privacy and Confidentiality

Limitation of Liability
Our services are provided "as is" and we make no warranties, express or implied, regarding the effectiveness of any treatment. Our liability is limited to the amount paid for services. We are not responsible for any indirect, special, or consequential damages.
Emergency Situations
In case of a medical emergency, please call 911 immediately. Our office hours are limited and we cannot provide 24/7 emergency services. For urgent non-emergency matters, please contact our office and we will respond as soon as possible.
Changes to Terms
We reserve the right to modify these terms at any time. Changes will be posted on our website and will be effective immediately upon posting. Your continued use of our services constitutes acceptance of any modified terms.
